ADHD Testing

A valid card on file is required to reserve your ADHD evaluation appointment.
Please give at least 24 hours’ notice to cancel or reschedule to avoid a $35 late-cancel/no-show fee.
Arriving more than 10 minutes late may require rescheduling and may result in a late-cancel fee.
Superbills are provided for clinical visits only.
Third-party diagnostic testing, re-testing, letters, and forms are self-pay services and are not included on superbills.
Creyos cognitive testing is always self-pay and is not billed to insurance, even if a patient’s insurance plan reimburses similar psychological or cognitive testing services elsewhere.
Payment for Creyos testing is due prior to test administration and is non-refundable once testing has begun.

Remote testing requires:
A private, quiet space
A stable internet connection
Use of a laptop or desktop computer when possible

1. Verbal feedback is provided at your results review visit.
2. Written summaries (if requested) are usually ready within 1–2 business days.
3. Follow-up re-test results are usually ready within 1 business day.
4. Letters and forms are processed within 5–10 business days.

For adolescents, a parent or legal guardian must consent to evaluation and testing.
Parent or teacher input may be requested when helpful to support diagnostic clarification.

Results are part of your medical record and are released only with your written authorization or as required by law.

Do you use ADHD rating scales
We use PHQ-9 and GAD-7 to screen for mood and anxiety and computerized cognitive testing to assess attention and executive function.
If an ADHD-specific rating scale would add value for your case, we can include one on a case-by-case basis.

How fast can I be seen
Appointments are usually available sooner than comprehensive neuropsychological evaluations.

Will this meet school or workplace documentation needs
Many patients use our written summary or letters to guide care and request reasonable supports.
Some schools or workplaces may require a full neuropsychological evaluation.
We will help you decide and provide referrals when needed.

Do you take insurance
Yes. We accept insurance for clinical visits when applicable.
The initial ADHD evaluation is billed as a standard clinical visit (insurance or cash pay).
Follow-up visits are billed separately as standard medical appointments.
